II. MAIN NOTARY FORMS

1. California Notary Acknowledgment: download form (CA Civ. Code § 1189)
Download, pre-fill the county, year, and notary's name, then print multiple copies for future use, or fill it out completely for a specific document.

2. California Notary Jurat: download form (CA Gov. Code § 8202)

Download, pre-fill the county and year, then print multiple copies for future use, or fill it out completely for a specific document.

III. ADDITIONAL NOTARY FORMS

1. Signer's Oath: download form (CA Gov. Code § 8202)

Download, pre-fill the notary's name in the oath section, county and year in the jurat section, then print multiple copies for future use. Before handing it over to the signer, check the document-type checkboxes and fill in the property location (city and state). The signer will then fill in his or her name, date, and sign.

2. Oath of Credible Witness: download form (CA Civ. Code § 1185[b][1 & 2])

Download, pre-fill notary's name in the oath section, county and year in the jurat section, then print multiple copies for future use. Before handing it over to the credible witness, fill in the name of the person whose identity is being sworn in item 1, after "known to me as." The witness will fill in his or her name, initial items 1 through 7, fill in the address and date, and sign. 

3. Power of Attorney Copy Certification: download form (CA Prob. Code § 4307)

Download, pre-fill the county and notary's name, then print multiple copies for future use, or fill it out completely for a specific document.

4. Request For a Copy of a Notary Journal Line Item: download form (CA Gov. Code §§ 8206[c][e] and 8206.5)

Download, pre-fill notary's name, mailing and email addresses. The requester will take care of the rest of the form.

5. Witness Affidavit for Signature by Mark: download form (
CA Civ. Code § 14[a])
Download, pre-fill signer's name, document title/description, date, witnesses' names, as well as the jurat part, or you can just print a bunch of them to fill out at the signing.

IV. SUPPLEMENTARY FORMS

1. Notary Non-Attorney Acknowledgment: download form 

Download, pre-fill notary's personal and business names, then print multiple copies for future use. The signer will take care of the bottom section.

2. True Copy Affidavit: download form

Download, pre-fill the county and year in the jurat section, then print multiple copies for future use. The signer must fill out the affidavit section in its entirety—DO NOT do that yourself.

3. Name Affidavit: download form

Download, pre-fill notary's name in the affidavit section, county and year in the jurat section, then print multiple copies for future use. Before handing it over to the signer, fill in the A.K.A. name in the affidavit section, after "a notary public"; the signer will take care of the rest of the affidavit section.
